I get asked this question often during my daily interactions with patients. The answer isn´t really as clear cut as it seems. It all depends on what exactly are we treating. Below I try to answer this elusive question by symptom: I used the book by Nelson Vergel: Testosterone a Man´s Guide as a reference.
Libido: Sexual interest usually starts about 3 weeks into therapy and it plateaus at around 6 weeks.
Erections: It may take up to 6 months to reach the full benefit of the hormone.
Quality of life: You start to see positive changes as soon as 3 weeks.
Depression and/or moodiness: Improvements in this area start at 3 weeks and reach full effect at 30 weeks.
Erythropoiesis: Red blood cell production increases at 3 months and peaks at 9-12 months.
Prostate specific antigen and gland volume: It can increase a little with a maximum plateau at about a year. I recommend at least a basal reference value at the start of treatment and a control value 6 months and 1 year after initial treatment.
Lipids: Positive effects appear as soon as 4 weeks with maximum benefits 12 months down the line.
Fat mass, lean body mass and muscle strength improves within 12 to 16 weeks and continue to improve throughout the years.
Reversal of inflammation occurs within 3 to 12 weeks.
Positive effects on bone mass are detectable at 6 months and continue for at least 3 years after initiation of treatment.
