Hormone replacement with pellets has been successfully used in men and women through the United States, Europe, and Australia. The first testosterone pellets implanted date back as far as 1938. Many are surprised to learn that pellet implants were one of the more popular hormone replacement forms used in the United States up and until the 1970s. That is the time period when oral and topical delivery systems were introduced. However, over the last 10 years, bio-identical hormone replacement has once again become popular. Partly due to the negative side-effects of oral hormone replacement in men and women.
Evidenced-based research data clearly shows Pellet Implants as one of the more convenient and more reliable forms of testosterone replacement therapy:
- Pellet implants deliver consistent physiological hormone replacement levels for 3-5 months in most people.
- Testosterone replacement using pellets improves bone density in aging men and women.

Usually pellets are inserted in the buttocks or in the lower abdomen using a tine incision not much larger than a small cut. Pellets are very small – usually between the size of a grain of rice and a tic-tac (mint). Pellets will generally last 3 months, but in some cases up to 5 months. They are inserted under the skin using a trocar (a small device that enables the pellet to be pushed below the skin). The incision is closed with sterile strips. Patients are asked to avoid heavy physical activity for 2-3 days to give the pellet time to settle in and the skin to close and heal. Patients should absolutely avoid swimming, but showers are okay.
Overwhelmingly, most men who have done pellets prefer pellets. It is the premium mode of testosterone delivery offering great results and lower side effects. More and more physicians in the United States are re-learning the advantages of pellet implant therapy. Those physicians that avoid it, usually do so because they are not trained well enough in the implant process and prefer the simpler approach of just writing the prescription on a piece of script paper.
